It provides a shared care management data platform in which a patient’s care management information can be viewed and updated by both ECM care managers and community-based providers. The app provides mechanisms to determine which health problems and health interventions impact a member’s health and well-being.

Through the use of APIs, full health data exchange between ECM providers and community-based providers own data ecosystems may be completed.
